VALIDITY OF MEHRAN RISK SCORE FOR PREDICTING CONTRAST INDUCED NEPHROPATHY IN MODERN PRIMARY PERCUTANEOUS CORONARY INTERVENTIONS ERA

Objectives: Contrast induced nephropathy (CIN) is a common complication and found to be associated with increased morbidity mortality after primary percutaneous coronary intervention (PCI). The objective of this study was validate the Mehran Risk Score (MRS) for risk stratification CIN in patients undergone PCI. Methodology: A cohort consecutive PCI at tertiary care cardiac center were included study. Patients Killip class IV presentation, patents history any PCI, chronic kidney diseases excluded from MRS calculated baseline post procedure serum creatinine level increase either 25% or 0.5 mg/dL taken as CIN. Results: total 547 included, which 79.3%(434) male. observed 62(11.3%) patients. area under curve (AUC) 0.712 [0.641 0.783]. Cut-off value ≥6.5 had sensitivity 61.3% [48.1%-73.4%] positive predictive 21.2% [17.5%-25.6%] specificity 70.9% [66.7%-74.9%] negative 93.5% [91.3%-95.2%]. an independent predictor on multivariable analysis adjusted odds ratios (OR) 3.86 [2.23-6.68] along multi-vessel OR 2.31 [1.27-4.19]. Conclusion: has shown have good discriminating power. However low optimal cutoff prediction suggests need modification improve its clinical utility modern era
